Each year, Members' enrolled in the Health and Dental Plans have the option of adding coverage for either or both plans for spouses and/or dependents. For insurance purposes, dependents include children, spouses, same-sex and common-law partners. Parents and siblings are NOT considered dependents.
Deadline for Winter term only students just starting classes in January is Friday February 3, 2017 at 6pm to add dependents- spouse/partner or children at the Member Service Office, located in the Student Centre Lobby, 55 Gould St. and pay the family coverage fee. We accept CASH or DEBIT only.
There are four packages to chose from. You must complete the form in person, ADD-ON forms are available starting MONDAY AUGUST 29 at the Member Services Office.
All family add-on forms must be received by the same deadline for the semester period of enrollment.
You can add family members to either only Health or Dental or both, there are 4 packages to choose from depending on number of people.
If your eligibility for membership in the Plan changes while you have family coverage, you should contact the Members' Health and Dental Plan Administrator.
The prices for Winter 2017 are as follows:
HEALTH PLAN ($90.00 for an unlimited number of dependents)
DENTAL PLAN ($120.00 for 1 dependent or $240.00 for 2 or more)
HEALTH AND DENTAL PLANS ($210.00 for 1 dependent or $330.00 for 2 or more)
HEALTH (unlimited dependants) AND DENTAL (1 dependent) $210.00
RETURN FORM WITH PAYMENT (Cash/Debit only) IN PERSON
